Upgraded Patient Forms System

We've completely rebuilt the patient forms system from the ground up — making forms easier to build, more powerful for patients, and better integrated with the rest of Clinically.

What's new

Drag-and-drop form builder

Design your forms visually with a new drag-and-drop editor. Add fields, rearrange them, configure validation, and preview — all without touching code or templates.

Healthcare-specific field types

New built-in field types for Australian Medicare provider numbers, health fund selection, phone numbers, and mobile numbers — with proper validation and formatting.

Multi-language support

Forms now support 15 languages including Chinese, Arabic, Vietnamese, Greek, Korean, and more. Use one-click AI-powered translation to convert all your form labels, help text, and placeholders. The correct language is automatically detected from the patient's browser, or can be set per-template. RTL languages like Arabic are fully supported.

Per-tenant branding

Customise your form colours (primary, background, and surface) from Settings so public-facing forms match your practice branding. Your organisation logo is included automatically.

Smarter submissions

  • Provider lookup mid-form — When a referrer enters their provider number, the form automatically looks up their details and pre-fills known information.
  • Variable mapping — Map form fields to patient, referrer, and clinic data so submitted information flows directly into the right records.
  • Patient data sync — When patients complete forms, their details (name, contact, Medicare, address) sync back to their record automatically.

Post-submission options

Configure what happens after a form is submitted on a per-template basis:

  • PDF download — Patients can download a copy of their completed form via a secure, time-limited link.
  • Email copy — Automatically send the patient a PDF copy of their submission.

Workflow integration

Trigger patient forms directly from automation workflows — for example, sending a health fund details form as part of a triage process. Daily reminders are sent automatically for incomplete forms.
